We are proud to be a paperless company.  After booking an appointment you will receive an email notification that will contain a secure link to a health history form.  Please fill this out prior to your first appointment to ensure you get the most thorough experience with your practitioner.  

If you have private health insurance, you may fill out the information on the health history form or bring it with you when you visit the studio.  We encourage you to check your health benefits to ensure your coverage and to discover if you require a physician's referral for treatment.
